1. The overall description sounds like a memory leak. You could try using Cleanmem to try and fix it. Not guaranteed to work as something else could be causing the problem.
2. Maybe there isn't enough space on your HDD for the game to run properly. Having free space is essential for programs to run as they should. It's why it's always recommended to keep your disk out of the red, keeping as much free space as possible. Worth deleting heavy games you haven't played in months if you're running out of space.
3. This might be a tough one to read, but from my own experience, you may need to upgrade at least your RAM, though there's a chance that might not be enough. I have an i7 @ 2.80 GHz, 1050 Ti, 16 GB of RAM, and also a 1 TB HDD. The 16 GB of RAM is an upgrade I made months ago. I own a Dell laptop which shipped with all those specs and 8 GB of RAM. Even with everything on minimum and the FPS locked at 30, Watch Dogs 2 would lag after a while and I could not drive too fast or else I'd get some serious stutter. Uplay would also always inform me I didn't have enough RAM to run the game. It happens as background processes (especially on Windows 10) eat out resources, some times more than they should, some times at random (again, especially on Windows 10). As soon as I upgraded to 16 GB, not only did I not have any trouble running the game, but I now have the graphics set to a mix of medium and high. I still have the game locked at 30 FPS cuz this is one of those cases where it doesn't feel horribly choppy and I'm fine with that. I try to be realistic and I don't want my computer to explode because I've been playing WD2 with medium to high graphics settings and 60 FPS. All other Ubisoft games I own that I have been playing recently (TD, TD2, GRW, for example) also run flawlessly with a similar mix of medium to high graphics.
4. And finally, it's always worth running a malware check whenever something isn't running properly. I myself use Malware Bytes for these periodic checks and it hasn't disappointed yet.
