Windows is intalled to a seperate SSD, and both use NTFS filesystems. I've tried both running Steam as an admin and disabling my av (Avast). I've health checked the SSD with Crucial Storage Executive, can't see any problems... Also have done a full format - no change.

If you are using Defender there anyway, then uninstalling your third party antivirus program (if you have one installed) should not be an issue.
I assume by this reply, you never had one then at all, unless I misunderstand.
There can still be other programs that can cause this. But yes, I would still have exceptions for real-time scanning in Defender. It's not supposed to mess with Steam and games anyway, so therefore exceptions are just added insurance it won't.
No security program whatsoever should be messing with Steam. Including Windows Firewall. They all need to be leaving Steam alone.
If you want to figure out if it is software related, then you can use safe mode with networking to test Steam downloads. However, this method may take more time and be more involved.
If the issue goes away while downloading in safe mode with networking, then it is a software conflict causing the issue in a normal boot, as mentioned above.
If you want us to look at possible programs that you have which we have seen be a problem before, and maybe help that way, then do this..........
1. Take screenshots of all of the programs you have installed on your computer. Control panel then programs/programs and features. (Windows 7,8,8.1) or "apps" (Windows 10 click "Start" (Windows icon on bottom left))
2. Click Windows Start and type in msconfig and run that resulting process. Go to the services tab and check the box on bottom "Hide all Microsoft services". Take screenshots of all that is left.
3. Then go to the startup tab on top of that same window (Task Manager in W10). Take screenshots of all that there also.
