It would help that you said what game it is...
Some are purely cosmetics, others are cosmetics as "supporter" to give the developer more to work on it, and expansive dlcs are "expansions" such as more missions to continue or finish the game in it's full progression, such as zombie army 4", take a look if you want to see an example on that store page.
So, it really depends on what game and what the dlc is actually for.
downloadable content is just added on stuff. you typically shouldn't need to to play a game its moreso extras or more content for said game. steam gives easy way to manage DLC in your library each game should have it's own DLC section if it has DLC content
So How do I get it?
DLCs are just extra content for game or cosmetics.
You can always review it and look it up to see if you might enjoy it.
You can buy the game and the dlc at the same time. Or, you can buy the dlc at a later time, adding it to the cart and checking out like a normal purchase. Then you just launch the game after it updates unless it already has the content but needed ownership of the dlc to access it.
It used to, but now i think there is a warning that warns you about not buying the dlc without base game on the store page and a popup window. Not sure how this person missed these messeges. Maybe they clicked their mouse too fast?
Once you possess the base game you go lower on the store game page and you'll notice that there is a big square listing those options out. Some game have absolutely none and some game have... Way too many. So if you want to add those options or DLCs to your base game you have to purchase them from the same game store page, as an extra over what you already paid.
DLCs are (supposedly) completely optional and one is (supposed to be) able to play the base game completely fine without said DLCs. There is however FOMO and also the fact that every game dev also alienate their users with time by altering the base game every time a new DLC go out with the pretence that it has to be this way so it work well with the new content... Many games end up a patently worse experience unless you actually buy those DLCs.
It could also be there mixing up what MODS are versus what DLCS are.
