They can choose to change their name to their Steam64 ID if they want to do so.
Anyone can do it.
That revert it to the Steam64 ID, yes.
Also, account termination does the same thing.

It is the default avatar for every new account.

Your account may not be deleted, hijackers will often times change the profile name to that of the profiles steam ID
