Yea, I've had this happen a few times. I've seen it happen once when a guy was out mining and came back but could get from the mining shuttle to the bathroom fast enough. I have also seen it happen if you try and put more than one bathroom in the same "room" because sometimes two crew memebers will start talking when they see each other in the same room, and then will stop heading to the bathroom when they start talking, leading to them crapping their pants.

So far I'm putting only one "bathroom unit" per walled-and-doored-in "bathroom" and I am putting several of them throughout the ship in question -- one by the common areas, one by the shuttlecraft, one by the factory/growing/etc, one by the storage, etc. You don't have to go overkill, but keeping them spaced around helps. It also really does help to only have one bathroom-unit per physical "bathroom" you build.

I also put a bathroom in their personal rooms.

Currently I'm experimenting with a 1-hour free-time slot in the middle of the work day to let everyone eat and go to the facilities.

Great idea, except if the guy is on an away mission too far away, he still takes a dump before he can get back to base.

Still, every bit helps.

Actually, if you see people on extended away missions, you might want to put a big rest break into their individual schedule to allow them to make it back with clean drawers.
