I'm pretty sure there's a difference between damaging him with an SMG and damaging him with rockets. I'm also fairly sure the game has four difficulties.

Kiting means you attack the enemy from a safe distance, and relocate before they ever have chance to retaliate. You can frequently swap between doing one or the other, or do both simultaneously.
For Tartarus himself you don't really need to swap between levels of elevation. You can quite easily crouch-jump over the barriers on the centre level as he gets close, which will force him to run the long way around, allowing you loads of time lay into him and choose your next move.

Edit: The spawn triggers of his friends are definitely tied to Tartarus' current health. Three or four waves. They always appear from the other platform that is identical to the one that Johnson is standing on. Check it every single time you damage Tartarus just in case. If you see them coming, grav lift to the top level and pop them in the head with that carbine.
