For starters, yellow with "..." means either partially equipped or claimed but not equipped right now.
In this case there is nothing to partially equip so it's most likely the later.
Red with a "!" means either no item is available or the person didn't manage to get one last time they tried.
The easiest way to force them to check again is to "assign uniform" again.
You want to avoid using that for those that have named their weapon/shield (unless you want them to not wear it) but in this case it's fine.
Make sure that there are proper pieces of equipment for everyone (with the right size if there are non-dwarves).
Replace clothes is usually a good option when you want full body armor, it prevents conflicts with clothes, especially for boots.
You might also want to check the schedule, it is possible that the active one is not set to "always wear equipment", meaning that they only try to equip things when they become active (through an order or training).
I figured out why some of the stuff was yellow. It was because a couple of the soldiers were in the hospital, waiting to be sutured, but I didn't have any thread at the time so they just kept waiting there and not picking up their equipment, which resulted in yellow icons.
Something else I noticed was that the soldiers who had red icons with a "!" were already wearing armor that was part of their civilian outfit. I tried giving them an outfit that replaced civilian clothes, but then they just didn't wear any armor at all.
After a bunch of fiddling I was finally able to get the buggy soldier to wear the correct armor by setting a uniform that replaced civilian clothes, requiring exact matches. For whatever reason, allowing partial matches kept resulting in them not wearing anything.
One other thing that's weird is that the soldier graphics in the squad menu don't match the actual soldiers' graphics. It shows them not wearing armor when they are, or vice versa.
But anyway, I was able to get everyone to wear the proper stuff, save for one soldier who for some reason won't pick up their crossbow bolts (it's still a yellow icon). Not sure what's up with that, but for the most part the issue is looking a lot better!
If the bolts are in a bin, get them out.
If you created their uniform after you created the squad, disband the squad and make a new one. And make new bolts.
