Make sure that there is no "Floor" on the tiles the waterwheel is on. It has to have freedom of movement in that tile. So, basically, a two-tile depth with nothing but water possible in the tile it's operating on.
These work even after digging 1 deep, eventhough i've read you need +2

It shouldn't be a requirement, but I see no other difference between my working one and yours.

Try digging another channel perhaps. Dig twice down and finish it to size before you connect it to your stream.
Have a bunch of perpetuum mobile power generation assemblies working that way without any issue, similar to the wiki's design (though I run with two more wheels and additional side channels for continued water flow) https://www.dwarffortresswiki.org/index.php/Water_wheel#Dwarven_Water_Reactor
My bet (have imgur blocked, so can't verify) is on the channel not providing a "flowing" water path as Thai Lizard mentioned.
Or could be some new bug with water flow calculations.
