If you have 10 dwarves of that profession, the guild will be established. Then you can choose if only guild members have acces or everyone. If the guild is not yet established, all dwarves have acces automatically.
Usually a "Farmer's Guild" is the first you will have established. You can set up a "Metalsmith Guildhall" quite early on. Any dwarf with a suitable skill may organize a demonstration of said skill. Everyone watching him will receive some skill gain.
I usually split my workforce in farmers and metalworkers and try to get those two guilds as fast as possible.
A "Doctor's Guidhall" is quite valuable too, as is provides some easy training for your medical staff.
