On the other hand, it has absolutely nothing to do with what you describe and to my knowledge they only changed how you designate them, not how they work.
An open central part was always going to be a potential threat, especially if any combat happens in that area, and that has not changed.

Yeah but there was/is a design in classic DF that while not eliminating the problem minimizes them to the point of nigh negligibility. I'll re-reply here after re-finding it.
Hatches do mitigate this, but anything that requires mitigation is defacto a risk inherent to central staircases.
This isn't a V50. issue though, it's been a thing for longer than I know. Though not many people know you can hatch an up/down stair case.
Never seen that happen in my 300 hours
and I always look at the combat log if something weird happens
Dwarves (or enemies) that fall unconcious can end up falling all the way to the bottom, which can be pretty deadly.
It's similar to some extent to how a dwarf can end up triggering traps if they fall unconcious on them (not so bad if it's a cage trap, usually deadly if it's a weapon trap that ends up triggering over and over).
If it is actually a thing, it's so rare that it doesn't require "fixing".
That said, I never said it needed "fixing" only that it, and brawls/other combat situation can kill or maim dwarves unless you mitigate the risk with hatches. It's still a risk, even if small. And players that haven been burned by it are gonna look for mitigation.
