Look at what the dwarf is wearing on the head, it is likely that there is a hood.
A hood is of the "cover" type when it comes to armor layering, meaning that it goes above even armor, so armor can't go above it.
It is different but similar to how anything besides socks conflict with high/low boots.
The easiest way to solve these issues is to tell the dwarf to replace clothing (in the military equipment) and to make sure the schedule is set to "wear at all times".
This means that they only equip armor and wear it even when not in active duty.
The downside being that until they get decent levels in armor user or high strength they will most likely be slowed down by the armor.
And I 'did' check to see if they had anything else.
It is something else.
My thought was maybe they needed a slightly larger size. like a human helm.
or perhaps they are a headless werebeast.. in disguise, i dunno.
