I loaded up an older save and he did not reproduce his escape, kind of unfortunately.
Although Rock Block Walls are REPUTEDLY supposed to impossible to climb, this is not the only report of similar things happening - and after all, siegers can ALSO climb rock block walls.
Either the Minotaur climbed or through some combat chicanery it got blasted up to the ceiling.
The simple solution to this would have been to build the arena bigger and ensure there was more than one tile between the edge and the hatches.
I have never read this. Constructed walls were always climbable. Smooth walls weren't, which makes sense if you think about it (https://dwarffortresswiki.org/index.php/Smoothing). There's also a difference between whether the wall was constructed with blocks or rough stones, as the latter is allegedly easier to climb.
I guess this needs some arena testing.

Sadly my minotaur died of depression.
