There's some labor settings that tell dwarves when and when not to load traps outside and in general. You want to go and check that out.
I had the same thing happen, undead came, I build traos - but my setting told dwarves to not load traps during a siege, so when I let the undead through the traps they walked right through.
The graphics difference between a loaded and non-loaded stone trap isn't opvious, so you can miss it the first time.
The pressure plate 100% is not working - it is also linked to the bridge, which must lift after something steps on it (the plate is configured for creatures of all size from humans to dragons).
Weapon traps - I don't understand how they work. They are loaded with serrated disks, blades, blowguns with blowdarts, spears, steel short swords. That kind of stuff. Seemingly they don't need to be linked to anything with mechanisms. Am I right?
The stone traps worked for me earlier, but in my new setup the beast just keep walking on them without any effect.
