My previous fort was a high boot nightmare, yet this one is free of issues.

Helm, mail, breastplate, gauntlets, high boots(just once for each...) Replace Uniform setting.
Mail shirt covers legs.
Dwarves don't have knees.

They get 2 gauntlets and high boots assigned. But they don't wear the high boots when with other (leather) shoes.
Ensure to "replace clothing" with "exact matches only" and make them always wear their uniform even when off duty.
If you modify the default "off duty" schedule to have them wear their uniform, it changes to "no orders" automatically.
What are you trying to do there?
You wear armor in layers, cloth, then chain, then breastplate (if you want that squad to have it), then a cloak.
AND if I try to correct it and allocate "Steel High Boot" twice in the uniform selection, they get THREE steel high boots assigned and put two on, with another somewhere in limbo that I can't find because NOW it says "2 assigned items" and "Steel High Boot". Sometimes they are carrying the third high boot in their hands if it's in their inventory, but most of the time I can't find it because it's one of the "2 assigned items", i/e you can't click on it to see the specific item.
I also can't make shirts in this playthrough for some reason (it's labelled as "Foreign", so the only torso coverage i can apply is dresses, which have a lot more coverage than a shirt, i/e you now have 2 layers of cloth on the legs (trousers/dress) so double crush damage reduction, but I THINK that because it also covers the lower legs that it is messing up the 2nd steel boot. maybe, i dunno. But that doesn't make sense because I CAN get them to wear 2 steel boots, I just have to accept the fact that they actually have 3 steel boots not 2. Because they wander around holding a steel boot all the time. Because.. reasons.
Your dwarves just getting assigned 1 high boot instead of a pair is propably a bug if they are naked otherwise and your uniform does not contain any other footwear.
It's weird anyway and it *should* work.
Maybe remove all steel boots from all uniforms, melt them all down and make new ones will help. I had similar problems with a single dwarf after I melted down a low grade boot he had assigned. forbid the one he was wearing and pressed "update equipment" in his squad settings and he got a new pair and I could unforbid and melt the single boot.
Try a simple uniform as written above, with "replace clothing", just to be sure your high boots do not interfere with _any_ other clothing.
And don't include "clothing" in your uniforms, because clothing does wear off and worn off clothing in unforms is not replaced.
And to answer your question.. they go and get 1 boot. Not 2. The only way I can get them to get 2 boots is to allocate boots twice.. then they go and get 3 boots. And they carry that one boot around when they are not hauling. and when they are hauling it disappears somewhere. Maybe into the backpack, I don't know. It's not strapped to anything or even shows up in the item list. I can't find them again because it's part of the "2 items assigned" so I can't see it again. It's not in their rooms, I checked.
I should iterate that these two things, the foreign shirt and the Das Boot thing is peculiar to just this playthrough.
Not sure what the issue is with your dwarfs though. Dresses won't be the problem eitherway; they use up torso slots, not feet slots. They wearing 2 socks on 1 foot by chance?
