"A diplomat could not complete a meeting and left unhappy"
No idea why. I just started my fort and in summer, I got this message. My trade depot was ready

It also happenned in one of my other fort where maybe on year 3 and 4, I got this message. Year 5 was fine.

Whats causing this?

I've not seen this happen, so can't confirm this is a mechanic. But is there any chance the 'diplomacy' button was on your screen for a while and you didn't click it, so eventually the diplomat left simply because diplomacy timed out due to in-game time moving?

nope I added a meeting area and he left again. (after reloading)

It arrives the same time that there is the big obvious "ALERT" warning when the trader arrives and I am always keeping an eyes on the alert (and constantly dismissing them)

All you should need is an office and the dorf responsible to have some free time to conduct the meeting

The diplomat is always trying to meet with your mayor, expedition leader or baron. These dwarves need to have an office assigned to them for the meeting (they can meet in the bedroom, but will get an unhappy thought about it), and the dwarf in question cannot be busy with other stuff.

The diplomat waits about a month or two before they leave.

When they are ready to meet, there's a purple diplomacy button that comes up by the red alert button, click it (don't dismiss it), to begin the dialogue.

If the dwarf is busy or not (at the moment the liaison arives) doesnt make a difference, and if you have no bedroom or office with your leader, the liaison doesnt wait, he is leaving the second you get the alarm.

If you don't actively complete the meeting yourself by clicking the diplomacy button and clicking through the dialogues, this happens.

Nerco-ing an old thread because I've just had this happen and this thread is the top Google result.

For me, the cause was my mayor being taken by a mood - looks like a known issue: https://dwarffortressbugtracker.com/view.php?id=622
