Missing dwarf's portrait picture in lists (possible bug?)

Hello,
I experience this issue quite often. When I view my dwarves in any list there are some with missing portraits / icons.
Does anyone know what's causing this issue?

same, and some of the weapon icons when changing weapons in uniforms.

I experience this only with wounded dwarves, weird. Reloading fixes it for a while.
