I had the same problem because I thought "oh yeah Urist is just gonna dig up and magically create stairs because this is a video game" when using the dig stairs menu.
I was wrong.
It's not a bug. You cannot dig into a ceiling. Marking a "dig stairway" order and going up will treat it as if you went from the top -> down but if the level is empty then no one can reach the order.
Once again, you have to BUILD the stairs on the same z-level your dwarf is at to go up.

Yes, this works because your dwarf is turning the block into stairs.
The automatic designation of both in this Steam version is nice, but it can only either construct or dig for all Z-levels, so we still need those separate stairs - perhaps as an advanced option as seen in the advanced digging menu. I hope this is addressed.
