Exception would be a waterfall, because these are usually flat at the top, so i dig down, and then make a huge multi-level feature around the waterfall, using it for mist.
But I building a fort's entrance and defences from the ground up on flat land has it's charm as well
Moat on one side and an archer gallery with carved fortifications on the other side of a 3 tile wide road is popular with me, too.
Combine with a couple of drawbridges for isolation, and you can make a convenient shooting gallery to upskill your marksdwarves with invaders. It takes a while, and a bit of planning to dig out the rock to do this... more if you want to have a pool for drowning anyone who falls off the road. And even more if you want to be able to drain the pool to recover gear...... but that's what this game is about.
Generally if you have enough dwarves and picks, by the time you're getting properly invaded the mining dwarves have hit legendary miner status and are big and strong and very effective military recruits.
Yeah that's why building everything from the ground up on flat land gives you some more control, then all you have to do is make some "lips" using flooring between some levels of the wall, and climbers will not be able to get in, also you can dig moats and stuff, of course you'll still be vulnerable to fliers unless you close all the top off.
As for when you just dig in a hill, what I usually do is not create any outside defenses, I create 2 entrances, one the normal one that leads to the fort as straight as possible, and then a secondary one, that leads to a deadly maze of traps.
which then I make lead to a big room, with properly placed fortifications one level above, from where my marksdwarves can soften the enemies that survived the maze a bit more before I send the infantry to wipe the rest out.
Then when a siege or something happens I just close the main entrance off and open the secondary, and the secondary has to lead to your fortress as well of course, otherwise the enemies will not path there
It's pretty fun, but it takes a huge amount time to clean everything afterwards...
Yeah, I've had dwarves sink into depression and wander aimlessly and start brawls from the shock of seeing all the dismembered bodies, and the stress of dealing with the miasma since it takes so long. It's possibly best to let everything rot first.
.... That and screen your clean up crew for mental fortitude using Dwarf Therapist.
Alternatively DF-Hack and Autodump is your friend.
Yeah I dont get that, dwarves should get happy thoughts when seeing a horribly disfigured goblin!
Miasma is indeed an issue though, gotta confess that at times I do a bit of cheating with dfhack, either using fastdwarf to have them do it as fast as possible os simply just deleting all the body parts and such.

There is something undeniably dwarvish about an impressive door into a mountain however...
Once, long ago, I embarked on a site that had to have been bugged. It was a sheer straight up and down dolomite cliff over 30 Z levels. It had no minerals or gems in it which is what makes me think it was a bugged embark. Still that massive cliff face was the perfect entrance to a mountain fort.
