The location of your base also matters, based on what insects' 'territory' you are in. Red Ants, for example, are more prone to getting angry with you where your base is at. Though you still do have to kill them, repeatedly, before a raid will start.
THose symbols you are mentioning are what I'm talking about, I miss them most of the time too. They aren't really in-your-face obvious though, which I figured is why. or maybe it's buggy and they just aren't always showing.
Your questions 3 and 4 aren't relevant, seeing as the chips have nothing to do with raids triggering.
The species that does the raid is based just on you killing that enemy type. The overall strength (quantity of enemies in the raid) is determined by your overall progress in the game and your gear, and also seems to be influenced by how often you've already been raided by that same species.
In my game, for example, I've been raided about 7 times total, and 6 of the 7 were orb-weaver raids. Because I kill spiders, mercilessly, any time I see one. The other raid was Larva, after I went on a larva killing spree.
I have not been raided by any other species, as I tend to jump around in terms of what I'm doing so never kill enough of the same type of enemy in short period of time to piss them off. I'm well past where you are in the game, and never have had any infected anything raid my base (yet). Probably because I never spend a lot of time in a row in the Haze killing things.
Since they tend to appear when you're in combat i'm not sure if they're bugged or if i'm just missing them most of the time either, but i've seen them a few times at least.
It's not like you really need them though since the cause and effect is so obvious - go on a X killing spree, expect X raid over the next couple of days.
In my experience raids mostly consist of standing around half the night and waiting for a handful of bugs to show up that may or may not come. About half the raids i've had nothing even showed up after the raid triggering interrupted me building or going to sleep. Really annoying.
I'm sure they can be challenging early game but once you're geared up they're mostly just tedious.
Sounds like the AI has trouble finding your base. My raids always arrive on time.

A few minutes later i get a "payback delivered" message and the raid ends, but so far i haven't found anything destroyed either.
They have ramped up though, yesterday Orb Weavers raided me and there was a total of 3 or 4 Orb Weavers and 5 Orb Weaver Jrs before the raid was over. I'm not sure on the total of the Orb Weavers because one of them just completely vanished while I was fighting it, It jumped up into the air and then was *poof* gone. I don't know if one of the ones that I killed after was the same one re-appearing or a different one.
My main base is elevated, but it has a foundation of walls and a few bridges, and I have a lot of storage of pallets and pet houses on the ground outside at ground level, so the raids always successfully reach me and do a little structural damage.
