It also means that the player did not perform "save-scumming". It is a way to cheat by not spending any resources/abilities until very last minute, and reload save if it is too late. If they did, they only deserve a gold variant.
Black Border or Black Medal means a lot, especially on Expert map to show that a particular strategy is highly reliable and less luck-based. Not only player from all skill level willing to understand when to spend, what is the major weakness of their strategy for better run; but also for game developers to make a better game balancing.
(such as increasing projectile speed, making ability less luck-based, reducing damage from spam-able defense, giving feedback on how a round is lost, rework certain upgrade, etc.)
Ultimately, it means that the player showed their dedication towards this game by spending more effort than usual, and deserved a unique title.

As Cat said, it's to award people who prepare thoroughly and don't just rely on scumming when their defences fail on a certain wave. It doesn't mean much on easier levels, but I can't even begin to imagine doing stuff like Bloody Puddles black border. The video doing it looks crazy!
