Glitch with map loading

I beat the game once. On my first play through in the hopsital level, I opened the padlocked door that leads to the underground facililty. The map did not load and I had to "find" my way to the door blindly. I could even clip through walls at one point. Second play through, back in the hospital again, at a certain point on the second floor, Carlos will stop abrupty and the map will only be partially loaded. The actual "map" when I press "m" will not load either. Has this happened to anyone else. Only seems to be in the hospital level.

Same bug with Jill right after the hospital, get to the safe room right before Nikolai and the factory area wont load at all.
