In Difficult Mode only, if you want to save your game you are going to need to use Ink Ribbons.
This system adds really a good tension and a SURVIVAL atmosphere in this game.
The relief when we were in the safe room was a touch from heavens and not just a vulgar check-point like in this RE3 doom remake...

Really? Only Ultra Hardcore is more "difficult" and have no impact on save system.

Your affirmation is two difficulties above Hardcore where is the proof of that?
IMO the game have just 4 level of difficulties and Hardcore is the level 3 (level max for a first game).
The difficulties listed are
Assisted, Standard, Hardcore, Nightmare, and ''?????'' (Yes, that is the difficulty stated.)
Also, I have Nightmare unlocked in my game.
It's a last resort but a pityfull solution. We can decided to do the game only with knife but it's a personnal challenge not a natural feature of the game.
It's very different.
And what about infinite save in this difficulties?
