Only other thing I could think of is it could be an issue with certain audio cards.
What do you use?, RealTEK has had issues in the past so it could be a RealTEK issue.
Also, I know there's a DTS setting on older Realtek versions. Checking the 'loudness' box should do the trick.

This didn't fix the issue for me, but it did point me in the right direction. It turns out that somehow, my connectors got changed back to line input and mic in, rather than my rear channels and center/subwoofer.
What fixed it for me was right-clicking the blue connector in the HD audio manager, selecting "Connector Retasking", and setting that one back to Rear Speaker Out, then doing the same with the pink connector, setting it to Center/Subwoofer Out. After doing that, the game sounds fine.
