For best graphics, it's a bit of a coin toss and dependent on preference. Across the board, they're both pretty similar, which is no surprise since I'm pretty certain the developers of both projects have worked together to make RealRTCW as good as it is, with RealRTCW also having used the Splatterladder HD Pack in previous versions, another great texture mod. The main difference graphics-wise between Venom and RealRTCW is that RealRTCW has reworked the weapon models and has more realistic skyboxes, while the Venom Mod's weapons and skyboxes are more similar in aesthetic to vanilla RTCW but better looking. If you wait a few hours, I'll put together a short compilation for you comparing the 2 possibilities for you to choose from. Those 2 choices are:
1) Install ioRTCW source port for higher resolution --> install Venom Mod --> set graphics preset to highest quality --> open the in-game console and type the following two commands to get rid of a lipsyncing bug specific to ioRTCW: "/s_useopenal 0" and then "/snd_restart"
2) Install the RealRTCW mod --> set graphics preset to Ultra Quality

/cg_fixedaspect 2 (unsquishes the HUD while in widescreen)
/vid_restart (required to lock-in /cg_fixedaspect 2)
/cg_fixedaspectfov 0 (allows you to adjust FOV while in /cg_fixedaspect 2)
/cg_fov 90 (adjusts fov)
/cg_gunz 1 (adjusts weapon viewmodel height)
