It seems like a bit of a project to get this up and running. I might give it a try. The map downloader seems promising, but I think a giant map pack would be better (didn't see one in the tutorial but maybe it was mentioned) since you wouldn't be constantly having the game close and re-open every time the server changes to a new custom map.
Most servers play vanilla or a little selection of custom maps, you should be fine

Also is there certain servers that people play on? I found some site that indexes servers and lets you filter out bots but it looks like most people are playing Enemy Territory and not this?
ECGN usually, but as pointed in the guide, you can use SplatterLadder to filter server
But if you everyone never joins, server will remain empty or only bots, so just hop in
RTCW had a terrible multiplayer that nobody cared about and only played for 3 months, 23 years ago. They made up for it by expanding greatly upon the multiplayer portion and then releasing it for free as Wolfenstein: Enemy Territory, aka ET. ET is almost dead but there's maybe 5 servers on gametracker that you can get a good game once every couple of days. You are wasting your time on the original RTCW multiplayer, it was popular for 3 months, 23 years ago, and ET is 10,000% better in every conceivable way
