Targeting Question on Multistrike

I've been trying to see if Multistrike is a viable option for my build (void warrior) and... I don't know if I'm seeing this right, but every time it looks like it just isn't actually attacking single targets.

I definitely see it when it attacks multiple enemies, but when striking a single large boss monster, it's just my single attack damage showing up.

Can anyone explain why this is happening? Is there a part in the skill tree that can assist? I'm looking for it to be my single target + refresh skill, but at this rate, might need to drop it.

Using Two-handed weapons should this be of any affect

Doesn't the tooltip specifically state the multistrike doesn't hit the initial target?

Apparently it does if you hit Alt while reading it.
