Cannot get pass Liath's Tower

I have been fighting the same boss in that Tower 3 times. And everytime I beat him. I couldn't continue because I couldn't pick that damn book. What is going on here ?

If you're using a controller, the newest patch seems to have changed the functionality of picking up quest items. Use your mouse on the object and you should be fine.
