Why? From what I have seen I would have to mark it up to coincidence. You may wish to think of it as just the roll of the dice, or it was just time for that encampment produced another unit. There are other factors as well, but I am not sufficiently expert to explain them.
I have had to use the strategy in the early phase of the game of sending two wariors simultaniously at any encampment, just to be certain that they can not kill one of my wariors. As you become able to produce stronger more ajile units, barbarian encampments become easier to disperse. Also, barbarian units do not heal, while you can heal your units.
Try attacking (preferably from rough terrain) until your health is half gone. Then rest (fortify) near the camp until healed. You will be attacked, but you should be able to defend a couple of attacks. This doesn't always work if there's more than two barbarians, of course. Also, sending your soldiers out in pairs makes it easier. The honor track makes barbarian fighting easier as well.
Edit: One more thing, a camp will only spawn one unit in a turn. If there's more than three units, there's another camp, or was until recently.
I usually attack the camp until we're both yellow or maybe red, fortify til healed, then finish them off. (Usually after I fortify til healed, it takes two more attacks to kill the camp). After this first attack, thats when the new barbarian spawns.
As for sending pairs of warriors, I don't usually make a second warrior til I get my worker unless I play militarily. I usually make a scout first. But even if I had a second warrior, I would rather use that to explore for ruins or defend my tiles than have to send him along with another guy just to kill a camp because of this annoying fact that they spawn one turn before I kill the camp.
It doesn't spawn a barb just because your unit is low on health. Just because you've had it happen to you doesn't make it part of the game code. I've cleared thousands of barbarian camps without a new barb spawning as I'm doing so. I've even cleared out camps with spearmen using a single scout, without any new spawns.
One thing it sounds like is you're letting your health drop too much. If it's a lone warrior, don't go below half health unless you know you're going to clear the camp. Be patient with it. It is ultimately cheaper to take a few more turns to clear the camp, then to replace a warrior. Especially early on.
I still say, use pairs. It takes longer to build, or uses up gold, but it's worth it. Send your scout to explore, and have them avoid barbarians if possible. They'll get ambushed anyway, but keep them away from encampments and you'll be okay.
I doubt the code says "you have 1 HP, spawn a barbarian." If you want to study a barbarian camp's spawning, try putting a group of archers two tiles away. Kill all units but don't disperse the camp. Any time something spawns, kill it. Then you'll get an idea of how they spawn. They might respawn the next turn, or they might be empty for several turns.
To ensure safety, keep your initial warrior at home until you get an archer out. Archer/warrior combo can take out barb cambs with extreme ease.
