So lets talk about the scaling
Because there are some unanswered questions, and I'm wondering if anyone has done the exact math for this.

I know from looking at various youtube videos that certain things regarding combat (and sometimes drops) scale depending upon how many players are present within a game. Namely how much damage each individual player takes, and seemingly how much health enemies themselves have or how much damage they take, can't tell. Those were from games with 8+ people though, in comparison to solo worlds.

Exactly how is the scaling applied though? Is it applied per players CURRENTLY present in the world itself, within an active area (something similar to ugrids?), or is it applied per the amount of players that have visited said world at all, active or not?

Some part of me is starting to believe it's that last one after observing some weird behavior in a solo world. Invited a friend just to check it out and look around for a bit, he didn't stay for long, but afterwards I noticed scaling still seems to have gone up, even after logging out and back in. Can't be sure though, I haven't run the numbers.

From what I've read, the scaling is based on active players (logged in), and there is also a range limit (so if you're far enough from everyone else, the enemies don't scale).
What does scale is the HP and damage of the enemies, rather than the damage you take. If you get smacked in the face by an enemy, I guess it's all the same, but if you cause damage to yourself (like falling or stepping in a fire) I don't think it scales.

officially its a 5% per player increase across the board. as Earl said, it only affects the mobs HP and Stamina.

Damage the enemies do does seem to change with more players. This does effect the damage you take and also can cause you to be staggered when normally you wouldn't get staggered by a hit solo.

I assume the player stagger effect is caused by amount of damage you take.

I have also seen some odd things in terms of my damage seeming to change for no reason even in solo worlds. For example there are times when axing a tree and enemy at the same time will alter my tree cutting damage for a while. Not always though, seems inconsistent and seems to go away either after a time has passed or if I portal.

Well the game is in EA so I am sure there are lots of little things like this that will get fixed over time.
