Ran msconfig
"Startup" TAB > clicked "Disable all"
"Services" TAB > clicked "Hide all Microsoft services" THEN "Disable all"
Clicked "Apply", then restarted the computer.
I suspect some services / background processes were conflicting with D3D (running "dxconfig" from the start menu would always crash -- you might want to test this).
I'll try MacTomb's solution in a bit. I had it pre-loaded, then I uninstalled and redownloaded the whole thing today.
Msconfig, basically all you do is press windows key, type in msconfig and press enter. If you're not running Windows 7, hit start menu, click run, and type in msconfig.
Have you tried slowly adding service by service to see which was causing the problem? Or re-enabling all services and seeing if it is still broken?
Fixed it by rolling back 3D card drivers pretty far.
