Death in 1999 mode
So I've just started a 1999 run. Died a couple of times already !!

After the last death I got a warning that I didn't have enough money to be revived should I die again.

Firstly, how much do you lose each death as I've never noticed the amount ?
Secondly, what happens if I die when I don't have enough money ? I'm guessing it's a restart from last save/checkpoint ? Surely not from the very beginning ?

From the wiki:

"On Easy Mode, death costs five Silver Eagles, on Normal Mode, the price rises to twenty-five, on Hard Mode, the cost rises again to fifty, and on 1999 Mode, every death costs the player a whopping 100 Silver Eagles. Moreover, if the player doesn't have enough money to respawn in the latter difficulty, death will result in a permanent game over."

Thanks.

I seem to be dying any time there's close combat. If I can keep them away from me then I'm OK.

But I think I miss spent my Season Pass infusions. I put them all into salts. But now feel they should have gone on the shield. I have just reached Elizabeth so can't see me re-starting now.

I think it's safe to say you done want to partake in close combat in 1999 mode. My runs consist on sniping from far away, and weeding some of the enemy.
