In Resident Evil 3 I think, she said guided. Her mission was getting G Virus. Due to her failure, Wesker was almost left her in the city. I believe she was not aware of the nuke.
She didn't fail, Wesker was simply ignoring that Ada did manage to obtain another sample through alternate means despite Hunk escaping with one of the few samples Birkin didn't destroy.
