Also, since you never see how Ada survives in RE2, her fate was unclear so it couldn't have been confirmed at that point.

She seemed to, yet she shows up again later to throw you the rocket launcher (at around 1:40 in this video). Leon recognizes her (and it is the same voice) whereas Claire (who didn't meet Ada) is like "who are you??"

But at the time when RE2 was the last installment of the series, she was dead. Yes, she showed up to toss you the rocket launcher, so she didn't die through the fall or Mr. X beating her up. (Both is pretty unlikely...but whatever).
But right after the fight against the tyrant, the entire lab exploded and she was not on the train at that moment nor did she made it out of the tunnel.
And since there is no way, she made it out of the lab on the regular way....she cant be one of the survivors.
Heck...even the later explanations of her survival where pretty far fetched.
And even if. What is she gonna do with it? The ways out of the lab a very limited. She did not escape through the emergency tunnel and her gadgets wouldn't help her at the elevator shaft.
