HM destroyed in any way, by anything=your fault, you are steve's target (unless other enemy hits him, or gets his attention)
Just the rules of the game.
[Edit] I meant that shuffle wands could be reason- for example if you fired very few times, modifier type spells could not be chosen/fired and that would be reason behind spell progress not being saved.
Although until now I wasn't sure, and sometimes I thought yeah maybe I equipped it but not actually fired it, or maybe I fired it into the HM and that doesn't count, now there's for example the homing spells modifier that I used throughout at least a whole level and even that still doesn't appear. I never noticed perks or enmies not unlocking but spells sometimes won't
that's not true, you can get in-game progress even with Cheat GUI on and it applies to spells that require an unlock as well
