What causes the chest at spawn to appear?
The first few times I died there was a chest when I respawned, but I haven't seen it since and have no idea what caused it to appear in the first place.

Any insights?

I believe it's just a random spawn that can occur. I get it every so often .

In general I think the RNG in the game isnt working as intended(or perhaps it is?), I havent played a whole lot but so far the pattern I am seeing is that between deaths, not a whole lot changes.

First session I played I 100% of the times was able to find a wand with the Torch spell which afaik is needed to explore the dark unlit areas. The second session a day later, hours and hours wasted trying to find it so I could continue the exploration, I couldnt find it even though I explored almost the entire Mines layer and the different areas(though finally, hours later I DID luck out and find one).

So yeah, it wouldnt surprise me at all that if you got a chest when you respawned it would be there for a few more respawns before the RNG switches it up again, seems like thats how it currently works.

Its from a chest under spawn being generated when something blocks its spawn. When entities cant spawn properly they teleport to the closest open space directly above that point.

You can see this in action when enemies drop gold too close to walls and sometimes near stacks of powder.

Sometimes a chest drops in the holy mountain when you complete a zone without killing any enemies. So maybe the chest at spawn is a way of the game taking pity?
