which departments do you take over and what certainty level do you use?

I am currently growing my hospital and have like 60 patients a day, ortho with hospitalization, and general surgery only the clinic. I have all department set to take over the patients and i have a perfect record so far. I was considering not taking over ED patients and just only taking over ortho, ICU and general surgery patients for my sanities sake, but im afraid of misdiagnosis, but since i am strapped for cash (first campaign mission) I am reticent to set certainty to high because I know it will clog up my tiny labs trying to diagnose every patient. I also enjoy treating every symptom I find at least the ones I identity up until diagnosis and treatment of the main condition.

So I guess my question, what do you guys do? How much certainty? Which departments do you manage personally? I know there are so many ways to play the game, but I hate misdiagnosis and I hate not treating all symptoms but idk if i wanna manage 100 patients every day honestly

Max certainty and none, I just take care of some of the critical patients.
