Janitor not wanting to clean waiting room
It's a few game now i have the exact same issue with janitors, after sometime they stop cleaning a few tiles in my waiting room, ending with a dirty ground modificator and a huge hit on reputation with no way to deal with that.

At first i thought it was cause i modified the prefab, but patients and doctors use it correctly. I toyed a bit with it, deleting it and recreating it a few times and it seem as soon as i move one object it's going crazy sooner or later. It's not instant, it takes sometime.

I have asigned a janitor to clean this particular room and he cleans always the same tiles and skyp the really dirty ones, as if he wasn't seeing them at all. On another test i completely emptied the room except one seat and the janitor once inside it start to make circles inside without ever cleaning anything, earning a ton of XP but not solving the issue.

Really looks like a bug to me but if anybody knows a way to stop this, except destroying and rebuilding the room again (What will clean the ground temporarily) i'd like to know.

Is the whole room assigned to the specific department with a cleaning room? Maybe select the department and make sure the whole area is highlighted and have a cleaning room.

Also possibly, i wonder if patients/ staff are to frequent in that area that when janitors go to clean someone is occupying the space? Maybe that center piece is causing issues?

I have janitors in the department and i have them manually assigned with the manager too, just to make sure.
Yes there are many people in the room most of the time but i have checked it manually too. Had a janitor get in the room with only 1 patient waiting (So not much) and he cleaned 3 tiles before stopping, letting always the same few tiles dirty. And what is fun is that he sometimes clean between those dirty tiles ...

Alright looks like you were right, putting more janitors manually asigned to the room finally got it clean when there was nobody inside. It's still a bit weird they needs to have the room completely empty to clean it but at least it works. (Sometime i see janitors clean a room while the doctor is in the middle of a dignosis and it does not seem to bother him lol)
