I've never had the need for more then 10 doctors offices in the ED to handle approximate 200 patients per day (counting walk-in/hospitalized). And my ED walk-in clinic is sort of inefficient, because it is staffed with 50% interns and only 1x 5* and 1x 4* doctor.
On their 12 hours shifts, doctors could receive 6 to 10 patients, as they also will want to eat and go to the toilet. So, for my part, I usually have some 20 doctors for 200 patients a day ; as I monitor all my patients, I can also skip some tests and treatment to go faster, but it becomes less and less easy the more departments are open (they won't have a heart attack if cardiology is not open).
There is also the possibility for you if you have other departements open to adapt the parameters in the tweakable file to have more patients going directly to other departments waiting rooms : in C:\Program Files (x86)\Steam\steamapps\common\Project Hospital\ProjectHospital_Data\StreamingAssets\Database. You can edit and change in the file Tweakables : <GameDBTweakableInt ID="CLINIC_REFERRAL_PATIENTS_PERCENT"> <Value>50</Value> </GameDBTweakableInt> ; the higher the value, the less you will have patients in ED waiting room.
