They are ... just the priority for the cleaner is to clean his department first and than he goes to the others.

They can work on the entire hospital, the janitors clean the department where they are assigned to first and then they clean the rest of the hospital. If you have enough janitors they can clean the entire hospitals

I like the way this works. In many games they all march to the same bit of trash like an army

I use the janitors that Im building for. If Im building a Nero surgery..then the janitors in the area will be nero..less switching pages. But, I was having a blood issue in my clinic. The surgery janitors never seemed to get the time to go to my ground floor to clean it. So, I tried adding "surgery" corridors..But, they still didnt make it down there. (I painter over the blood) I had to add a couple surgery janitors to my clinic..Just for blood..
