Not at the beginning of the game. Later on : Yes. ATM I make most money from hospitalization. But to start hozpitalization is very expensive and you don't have ambulances yet.
Also clinic for Neuro and Cardio gets you patients who pay 40 % more as your standard insurance company. Thats an awfull lot of money, you don't get when you didn't open it. Those % you will also miss on your hospitalization departments with the expensive treatments.
Spoiler, No you don't have every clinic room. If you only get 30 people you have only build EMS clinic and maybe GS clinic. You can however build GP offices and waiting rooms for the other departments. After you have hired staff for it, you can open the insurance companies for it. I would start with Cardio and Neuro as those insurances pay the most and have the best earnings a treatment. Kep in mind at some time you must also open Labs, toilets and cleaning rooms for those departments.
At the moment I'm at ingame day 16 and opened only 3 hospitalization departments yet. The last one I just opened.
You don't - you can have a doctor's office and waiting room and nothing else and it'll function just fine.
Yes, eventually you have to. for example not every lab examination is available at every lab. Also at the moment you hospitalize somebody (not EMS) you can't sent them back to observation. Also its a lot of manual work to keep them transfering to departments. Also Transfering patients costs extra time you don't want to loose that time with critical patients. In the beginning its a good solution to transfer patients for lab examinations, cause labs costs a lot of money. But eventually you want a own lab for every department. I wouldn't advise to build an hospitalization department without building a lab for that department first.
As that lab needs a waiting room, I would advise to keep it in mind for your future building plans.
Labs also don't need their own waiting room...
We are talking about making a profitable hospital. If you want to transfer 30-40 patients a day, be my guest. I rather spend my time otherwise, and the time of my staff otherwise.
I never said that labs needs their OWN waiting room. I said it needs a waiting room (or only use it for hospitalized patients). You want that lab close to a waiting room, otherwise your staff keeps waiting for the patients to bring their samples and to get their results.
The whole thread is about making a profitable hospital. Labs are not essential to this.

Essential for (let's be generous) 10% of cases you get (let's be generous) 20 patients a day... about 1k a treatment... that's 2k revenue from the lab.
100 for a technologist - that's 1.9k a day (let's be generous). Pays for itself in 8 days and (let's be generous) earns you 1.9k from then on.

Nope all clinics are unlocked from the start. Its only the hospitalization rooms that are locked.
