How do you get higher star rating?
I'm tryna increase my star rating for my hospital, yet it's stuck capped at 3 and a half star. I have all wings built to an extent. Small little wings, but they are there except for the ICU I haven't quite figured that out yet.

I have a massive cleaning staff, doctors everywhere (i only hire the expensive high perk people) when I look at my patients that are here or leaving almost all of them have a 5 star opinion. Short wait times, nice staff, etc.

Yet the overall rating wont go up so I am not getting the 30 people I need for this insurance contract to send more people. I've got potted plants lining my hallways and nice glass doors in all of the right places. I can't help but feel like i'm missing something super obvious that i'm just not seeing atm.

Do you have common rooms?
How many patients are untreated a day?
Are you able to treat patients fully?
How many patients get transported to other hospitals in a day?

Yea i find this entertaining, i use only lvl 2 doctors that are the cheapest and have 5 stars constanly lol

I have common rooms. It was a one point 2 patients a day, but I got myself back down to 0. According to the patients they think they're being treated fully. Most of my patients are literally just broken bones and other easy to diagnose things.
Patients have left but not been transported yet. I haven't been lucky enough to get a critical patient yet.

Patients leaving can have a really big impact on Prestige (star ratings). Try working on that and see what happens :D. Ideally you should have 0 untreated patients. Specially if its still a small clinic/hospital.

oof alright. yeah it's not huge. It only has like 3 medical doctors, and 1 for every specialized except for ICU because I have not unlocked that yet. I think I'll just scrub the save and start a new hospital, but do it better this time, now that I know what to focus on, and what not to focus on.


Speaking of which how do you unlock ambulances/ICU?

Yeah I tried that, but then I kept having patients come back with incorrect diagnosis so I was like eff it, only going for the best of the best to start out.

Ahh. Go into Management Mode and set the "Certainty" to "High". When you start, it is at "Medium" by default. When it is set to Medium, when you are down to 3 to 2 diagnoses, the doctor will not run anymore test will choose the most likely diagnosis (which isn't always correct). Even when it is 51% and 49% likeliness, the doctor will choose the 51%.

It won't matter how good the doctor is unless the certainty level is set higher.

Everything should get better when you set the Certainty to High :D

There is an insurance company objective that will let you unlock ambulances and ICU. Although I forgot which insurance companies haha. I think it was the 3rd one.

In my experience changing the certainty doesn't stop doctors guessing, it just means they will work to uncover every hidden symptom so you get the incorrect diagnosis popup before they are treated. If you sometimes have patients leaving setting the certainty to high will really slow things down and probably increase that.
I leave my departments on medium and the only doctors who ever guess (correctly or not) are lower level (3 & lower) doctors with diagnostic traits. Nobody else ever diagnoses before 100%.

But yes, the ambulances unlock in the Liver TY objectives, after setting up the ICU I think.

Don't forget to keep an eye on the department info too (button in the bottom left) if you're looking into prestige issues. It'll list the most common satisfaction modifiers for both patients and staff.

I guess it's time I go for the level 3 and lower to start out. It'll also make things easier with the lower wages. I got frustrated at one point, and took over the diagnosis for a couple of doctors because I found that I could usually find the illness better by performing all of the necessary tests. I went through the tutorial last year, and forgot a lot of what it taught. I guess I might need to go back through it, and figure out some of the you can alter that settings I forgot about lol.

Do you guys use high level Radiologists and other similar people or low level nurses and radiologists too?

I use to always hire lvl1 doctors but ive moved to lvl 2 have just hiring fewer, i try to keep their salary below 240 but if they have loyal and are close that salary ill hire them as they will level cheaper than others.

I haven't really felt the differences between high level and low level radiologists. I always hire the lowest level and wage staff when starting. But you'll get to a point later on where you can hire the best staff and it will barely be a dent on your earnings haha

I started a new hospital, and make it a little more efficient. I am doing a lot more multi floor planning. Basically a floor for each department sort of. More like 2-3 departments per floor. Trying to compact everything. With big huge nice waiting rooms with plenty of plants and sofas around for people to sit on their phones lol. Lots of glass to, personally I love the look of glass everywhere. But hospitals where I live also are all glass so it's probably because of what i'm used to seeing.

So far with your guys help I have been 5 stars for the most part. I probably have too many emergency docs but that's okay i'm treating 40+ patients a day. With a patient influx of 120% it says. The only negative is they're bored but they always rate 5 stars so that seems okay. So thanks guys!!


The only issue now is that my save might be bugged as it keeps saying there is no TCU available yet I have 3 on call docs, and 2 TCUs set up. (they're one bed each) I'll have to spend some time figuring that out. This game is for sure not an easy one unlike most other management games. Oh and I should mention as i'm learning most of the centers i'm building are the games built in pre fab ones.

It should show what your TCU is lacking in the management mode. If not, you could send us a screenshot so we could see whats going on :D

I highly suggest making your own prefabs as default built prefabs take up so much space but I guess its part of learning the game :D
