Lack of characters as the game goes on.
I've noticed in my Sicily campaign that the great grandson of the initial ruler couldn't be married because my huge country had only 4 eligible women, with the youngest of them being over 50.

Someone forgot to code in the character generator so after a few generations you're literally left with no choice but to marry into your own family like an Egyptian royal.

ok so, you have to integrate other cultures to 'citizen' in order to be able to marry any character from that culture.

I imagine they won't fare much better since all cultures seem to be subject to the same coding. But integrating cultures isn't really viable for long since integrated happiness falls for each extra culture you integrate, so I like to convert and assimilate them instead.

Still, what a horrible way to lose a game. Literally no women left to bear children.

On your culture panel you can take a decision for your main culture that allow you to adopt other characters. It's supposed to be for all big families but you can only manage those adoptions with your main dynasty.
If you need women, adopt a lad that have single daughters and sisters. If you need high character with high martiality, adopt a lad with a high stat (and high loyalty).
The trade-off for doing it is globally neglictable.
