You need to raise your Centralization which in terms help you raise your civilization...

Once you have high enough Centralization and Civilization you can reform your government with the Decision Tab of the UI

You also need to change some laws and have a certain level of tech. You should be able to see the requirements from the start if you look on the decisions tab though...

Depending on what nation you choose, you can skip the centralization crap and get straight to where you want to be. Some nations have decisions where you can form a new country with the bonus of a government upgrade; what nations are you wanting to play? I can tell you if they fit under the decision scope or not.

Here's the process. You need to get your civ level in your capital to 50. You need oratory 6 (or to border an existing monarchy). You need to enact the absolute authority law. First, make sure your citizens belong to your culture with the governor law "Cultural Assimilation". Once they're all your people, switch to "Civilization Effort" to begin converting tribesmen to the other types. Feel free to promote some people manually to citizens as well. Build markets.

Once you're at oratory 5 you can change the law to absolute authority in your government screen. Once you've done that and hit oratory 6, you can go to decisions and reform into a monarchy. Beware that doing so loses you the free upkeep on tribal retinues. This reformation will leave you much weaker militarily until you have the time and money to reform your armies.

So many thanks chaps.
I had completely forgotten the decisions menu!
I do appear to have a problem though. I am a Clan chief.
How to overcome this?
I am playing a tribe of Britain if that helps.

You're always a clan chief. That line means every character in your nation that isn't a clan chief must be loyal.

Then you're in luck! There's a decision that can expedite the process,

I'd wait until you have a good ruler with a decent lineage before enacting said decision. It's a fantastic way to skip the wait for civilization to rise, despite the tribes of that region being more numerous. If that seems like too much effort (which it might be, since Pritania consists of so many tribes and provinces - other options in different regions are much easier, i.e. Asturias, Aquitaine, and Aemorica), you can also speed up your centralization by enacting laws that relate to it - you basically just need to use the laws, or if their loyalty is too low, pause the game, imprison and execute the other tribal chiefs, and enact the decisions regardless of their approval.
