How to get Chaos is a Ladder

Draft Mode
Turn off all difficulty modifiers (You will need to be able to survive after picking mods to wait for the orbs to generate)
Try and re-roll until your first mod is Obsession, and reset if you don't get it near the beginning (Doubles all future recursive wild mods)
Get only the recursive mods, as they are doubled
Exceptions: Quantum Decoherence (+5 Levels) and Transmogrification (Helps avoid the chance of not getting any recursive wild mods)
When choosing recursive mods, aim for Chaotic Ambition (increases the amount of recursive wild mods offered) and Grandeur (+2 Levels)
Don't forget to also grab Discord.

Yeah I figured that that was the only way. Finally managed it after another dozen tries (got up to 69 mods, and only one non-wild). Research helps by letting you banish non-wild mods.

I ended up with like 8 health, managed to score almost 6000 points.

Challenger also works, just pick it up, fight the thing, then upgrade again. Best if you get it *before* Obsession (so you only have to fight one) but a pair early isn't too bad.

I got it without too much trouble just with a stealth + salvo build + just using rerolls conservatively and strategically to avoid getting stuck life-ending mods as opposed to fishing for slightly better choices. Note that it doesn't count picking the central gear choice as part of the requirement for the achievement, so you can pick whatever ship weapon shield combo you want at the start.

I think you're mixing it up with a different achievement. There are two chaos achievements and they seem to be talking about the discord orbs one not the pick the center option one.
