Are all the npcs out to scam you?

I’ve avoided being scammed twice but I did get scammed by the big boob lady. Should I be decline the npcs more?

Also I brought a contact lenses from the medical shop and I don’t know what to do with them.

Some people are legit and most of them are trying to scam you. If you refuse an offer and the substory doesn't continue, then it's generally fine to say yes.

I didn't remember what the contact lens did though.

The contact lens is just equipment. It doesn't appear to do anything other than minor defence when equipped.
