Edit: I've fixed mine. I have a surround sound capable sound card, and the output settings in the software for it was set to output to surround where it's all normally set to stereo.
Have a look in your system tray or control panel for your audio driver software and have a look there. Hope this helps.
I've just had a couple of big windows updates, and I think something may have reset my settings.
wondering if it's drawing properties from other devices than the primary for some reason.
Also make sure 'Allow applications to take exclusive control of this device' is unticked in the Advanced tab for your headphones.
