Missable achievements?

The last thing i did was kill the guardian ape and his lady friend, i haven't been tracking anything other than the prayer beads and i got the spear arm upgrade.

Game has 4 different endings so you will need to do some NG+ cycles anyway.

That said good idea to collect all player beads and "estus flask" in your first NG so you need to collect all items for the third ending.

Finishing all bosses and or prostetics also need at least 2 cycles so NG+ at least.

Finally getting all skills will take a while usually best done after all 4 endings(NG+3 are done).
