To be clear, you need to get a bell from an old woman (in area after the first general mini boss). Bring that bell to the altar inside the temple and you will be transported to a new level that is a memory of the past. The axe is near the beginning of that area in one of the small temples.

Very close to that there is also a cannon prosthetic near a bunch of warriors and dogs sitting at a campfire.

Go back to the guy to create the prostesis. Once you have 3 it unlocks a new skill tree, so better do that early.
