I think he is talking about the extra dialogue you can get from the Demon fight itself. There seems to be a way for Sekiro to realize who the demon is, but as far as I know no one knows what decides whether he does or not.
It's basically just Sekiro acknowledging it once during the fight and once a bit later if you talk to the old woman nearby.
Everything else, what other npc's say and what the old woman says is exactly the same.
