Now, there are FOUR Gauntlets of Strength - Divine Heir, Shura, Severance and Mortal Journey. If you have finished the game 3 times, Divine Heir should appear in your Gauntlet of Strength at the Sculptor Idol. The fact that you don't see Gauntlet of Strength at all, it suggests to me that you have started a brand new game from scratch. This will not work. You have to start a NG+ game if you want to start a new game. Divine Heir only has 3 bosses, Gyoubu, Lady Butterfly and Inner Genichiro. As long as you have completed the game (any ending of the four), Divine Heir will show up as you have bound to "meet" these 3 bosses first with any ending. Check at the Sculptor Idol. You should see Gauntlet of Strength there unless you have started a new game from scratch.

I had suffered the same problem. Back in 2020 and 2021, I have noticed that my Steam account had gone offline by itself even when I had no problem with my internet. So, Steam Client was a bit flaky at the time. But I have manually backup my Sekiro saved game files. I didn't get the very last achievement "Sekiro" for which I only had to defeat Isshin the Sword Saint. I did get the achievement for defeating Isshin the Sword Saint but it didn't give me the final achievement "Sekiro". I just restored the saved game files and fought Isshin Sword Saint again. And eventually, I got it.
So, were you playing the game with your Steam account offline? Please do not mix up with the in game setting of "Network Offline". That is just for those "remnants (purplish blue blobs) on the floor.
